If you still have doubts, check the Italian Ministry of Foreign Affairs Visa indications. In particular, fill this form to check if you need a visa and to obtain a list of required documents. Please note that after filling the form, you will obtain a list of Italian Consulates and Embassy in your country - these are the most reliable sources for visa information.
A Health Insurance is usually not required, but strongly recommended.
